What is a blepharoplasty?

Blepharoplasty, also known as eyelid reduction, is a surgical procedure to reshape the eyelids by removing excess fat and skin. It is a highly effective treatment for both functional and cosmetic concerns in the eye area.

Common concerns treated with blepharoplasty include: hooded or drooping eyelids, which may reduce the field of vision; under-eye bags, which can cause discomfort; skin laxity around the eyes caused by the natural ageing process.

Over time, a combination of skin thinning, muscle weakening, loss of elasticity due to collagen reduction, bone resorption and changes in fat distribution cause the upper and lower eyelids to sag. Loose skin accumulates as folds in the upper eyelids, which can droop over the natural eyelid and obscure vision. Relaxation of the lower eyelid muscle causes fat to push forward, creating the appearance of eye bags.

Why do people have blepharoplasty?

Medical reasons: to remove excess upper eyelid skin restricting peripheral vision or causing brow strain or headaches; to correct eyelid drooping (ptosis) that interferes with sight; to reduce lower eyelid puffiness causing irritation or discomfort; to restore normal eyelid function and improve comfort and visual field.

Aesthetic reasons: to rejuvenate the eye area by reducing drooping lids, brows and under-eye bags; to refine eyelid contour for a fresher, more balanced appearance; to enhance natural definition around the eyes.

Types of blepharoplasty

Upper eyelid blepharoplasty. Lower eyelid blepharoplasty. Double eyelid blepharoplasty.

Risks

Dry eyes — may worsen if previously experienced. Irritated or watery eyes — usually resolves within two weeks. Temporary swelling making eyes difficult to close. Blurred or double vision — rare and usually temporary. Dropping of the upper eyelid — may require secondary surgery. Eyelid retraction or ectropion of the lower eyelid. Haematoma or seroma requiring treatment. Asymmetrical eyes.

Frequently asked questions

What is eyelid reduction?

Eyelid reduction (blepharoplasty) is a surgical procedure to reshape the eyelids by removing excess fat and skin. It can be performed on the upper lids, lower lids, or both, and addresses both functional concerns (such as impaired vision from drooping lids) and aesthetic concerns (such as under-eye bags).

What side effects does blepharoplasty surgery have?

Common side effects include temporary swelling, bruising, dryness, and watering of the eyes. These typically resolve within two weeks. Less commonly, patients may experience temporary blurred or double vision.

What does blepharoplasty surgery involve?

Upper blepharoplasty is performed under local anaesthesia. The surgeon makes incisions along the natural creases of the eyelid, removes or repositions excess fat and skin, and closes the incisions with fine sutures. The procedure typically takes 1–2 hours and is performed as a day case.

What risks are associated with blepharoplasty surgery?

Risks include dry or irritated eyes, temporary visual disturbance, swelling, bruising, asymmetry, haematoma, ectropion (outward turning of the lower lid), and in rare cases, ptosis (drooping of the upper lid) requiring further surgery.

How should I prepare for blepharoplasty treatment?

You should attend a consultation with your surgeon to discuss your goals and medical history. You may be advised to stop smoking, avoid certain medications (such as aspirin and anti-inflammatories), and arrange for someone to accompany you on the day. Full pre-operative instructions will be provided.

Is blepharoplasty surgery painful?

The procedure is performed under local anaesthesia, so you will not feel pain during surgery. Some discomfort, tightness and soreness can be expected in the days following, which can be managed with over-the-counter pain relief.

How long does it take to recover from blepharoplasty?

Most patients can return to work within 1 week, though swelling and bruising may persist for 2 weeks. Full recovery, including resolution of minor swelling, takes approximately 2 weeks. Strenuous exercise should be avoided for 6 weeks.

Will my eyes look natural after blepharoplasty?

Yes — the aim of blepharoplasty is a refreshed, natural appearance. A skilled surgeon will work within the natural contours of your eye area to achieve a result that looks rejuvenated rather than operated upon.

What scarring can I expect following blepharoplasty?

Incisions are made in the natural creases of the eyelids, so scarring is typically well-concealed. Scars fade significantly over 3–6 months and are generally imperceptible once fully healed.

What are some of the genetic reasons for having eyelid surgery?

Some people have a hereditary tendency towards heavy or hooded upper lids, prominent under-eye bags, or ptosis — all of which may be present from a relatively young age regardless of the ageing process. In these cases, blepharoplasty can be considered for both functional and aesthetic reasons.
